Contact the Meritage Medical Network Administrative Office for the following:
- Authorization status
- Authorization modification requests – CPT/diagnosis/quantity changes, extensions, etc.
- Claim status – for large batches of inquires, spreadsheets can be faxed or emailed for a response.
- Questions regarding network access – who members may see in the network
- Eligibility verification
- Benefit details (copays, coinsurance, annual deductible, max out of pocket)
- Requesting copies of claim EOBs and/or authorization approval/denial notices
- Access to UM Staff for general questions and authorization issues
- Questions or issues regarding Meritage Medical Network’s authorization process, or how to obtain the policies, procedures, and criteria used to authorize, deny, or modify healthcare
